Position Summary: 

The Hospital Liaison plays a critical role in facilitating smooth transitions of care, ensuring that patients receive appropriate follow-up services after discharge, and serving as an advocate for patient needs. This individual will collaborate closely with hospital staff, external healthcare providers, and community resources to promote coordinated, patient-centered care. This individual will be responsible for growing census and focused on key accounts

Key Responsibilities:

  • Serve as the main point of contact between hospital departments and external agencies, including home health, hospice, rehabilitation, and long-term care facilities.
  • Educate patients and families on discharge plans, follow-up care, and available community resources.
  • Collaborate with case managers, social workers, and clinical staff to coordinate care plans and ensure continuity.
  • Assess patient needs and eligibility for various support services.
  • Maintain detailed documentation and track referrals, admissions, and patient outcomes.
  • Provide emotional support and guidance to patients and families during transitions.
  • Develop and execute comprehensive business development strategies aligned with company goals
  • Identify and pursue new market opportunities, partnerships, and revenue streams
  • Build and maintain relationships with key stakeholders, including clients, partners, and industry influencers
  • Collaborate with marketing and product teams to align go-to-market strategies
  • Lead and mentor the business development team to meet or exceed growth targets
  • Prepare and deliver presentations, proposals, and contract negotiations
  • Track key metrics and report on pipeline activity, performance, and ROI

Qualifications:

  • Bachelor’s degree in Nursing, Social Work, Healthcare Administration, Marketing or related field (required).
  • Clinical license (RN, LCSW, or equivalent) preferred but not required
  • Excellent interpersonal, communication, and organizational skills.
  • Strong understanding of healthcare systems, discharge planning, and community resources.
  • Ability to work effectively in a fast-paced, multidisciplinary environment.
  • 7+ years of experience in business development, sales, or strategic partnerships
  • Proven track record of driving business growth and closing high-value deals
  • Strong leadership and team management experience
  • Excellent communication, negotiation, and presentation skills
  • Ability to think strategically and execute methodically
  • Industry-specific experience a strong plus (e.g., healthcare, tech, B2B services)
